How CareCove works

One relationship. A clear process. Follow-through that does not stop at the handoff.

Annual Care is designed to connect the parts of elder support that often become fragmented — health observations, caregiver interaction, appointments, everyday needs, family communication and escalation when something needs attention.

Care that keeps life yours.

Annual Care · Care Manager

One person keeps the moving parts connected.

With Annual Care, your family has a dedicated Care Manager who owns the CareCove relationship — reviewing status, coordinating services, following open items, managing escalation and staying directly accessible to the family.

The Care Manager does not replace doctors, caregivers or family decisions. The role is to make sure CareCove’s part of the relationship remains connected and followed through.

A continuous cycle, not a one-time service.

CareCove 360° is the organizing process behind Annual Care — the method that supports continuity.

  1. 1
UnderstandWe learn about health, routines, lifestyle, preferences, everyday needs and family concerns.
  2. 2
AssessA structured Clinical Risk Assessment establishes a baseline across physical health, functional ability, nutrition, cognition and safety.
  3. 3
PlanSupport and follow-up are organized around identified needs and the older adult’s own preferences.
  4. 4
Engage & ObserveRegular caregiver interaction, health checks and agreed support create an ongoing view rather than a one-time snapshot.
  5. 5
Review & ActWhen a meaningful change or concern appears, it is reviewed and the appropriate action or escalation is initiated.
  6. 6
Communicate & ReassessActions and progress are documented, information is shared within the agreed consent arrangement, and support evolves when needs change.

When something needs attention

An observation matters only if something happens next.

When a meaningful change or concern is identified, it is documented, reviewed and routed to the appropriate next step. Every red-flag case receives medical review under CareCove’s medical-governance process, currently led by Dr. Tushar Kanti Das. This is not a promise of diagnosis or a guaranteed health outcome; it is a defined review and escalation process.

Family visibility, with the older adult’s consent.

For Annual Care, relevant observations and follow-up actions are recorded in CareCove systems. Designated family members can receive relevant WhatsApp updates and a consolidated monthly Care Summary, according to the agreed information-sharing arrangement.
