Independent, but the family wants structured local support
Your parent manages day to day, and you would like a regular, organised presence nearby.
Annual Care
Annual Care combines a dedicated Care Manager, structured assessment, regular caregiver interaction, health and everyday support, family communication and emergency/hospital assistance in one ongoing relationship.
Who Annual Care is for
Your parent manages day to day, and you would like a regular, organised presence nearby.
Several doctors, tests and follow-up actions that need someone to keep them connected.
You want to know what is happening without having to be there for every step.
Support and attention need to evolve as needs change.
You want one person to contact and a predictable pattern of care.
Dedicated Care Manager
Every Annual Care family has a dedicated Care Manager who continuously oversees the CareCove relationship.
Reviews your parent’s status every week, looking for meaningful changes, missed actions, emerging concerns and follow-up requirements.
Ensures scheduled interactions and agreed support are being carried out effectively.
Doctors, investigations, specialists, everyday assistance and CareCove services are connected rather than managed in isolation.
Meets your parent personally and gathers their feedback.
When something requires attention, coordinates the appropriate next step.
Your family can directly call or WhatsApp the dedicated Care Manager.
The Care Manager does not replace doctors, caregivers or family decisions. See how the Care Manager and caregiver roles differ
Clinical Risk Assessment
Changes in mobility, balance, strength, nutrition, cognition or general health often happen gradually. Annual Care creates a structured way to establish a baseline, notice meaningful changes and decide when follow-up may be needed.
The objective is not to turn ageing into a medical condition. It is a structured assessment and review process; it does not guarantee diagnosis, prevention or recovery.
Health history, vital signs and relevant clinical observations.
How comfortably and safely someone moves through everyday life.
Functional changes that may indicate increasing difficulty or risk.
Weight, nutrition-related changes and other relevant indicators.
Structured assessment where appropriate.
Factors in the home environment that may affect everyday safety.
CareCove 360°
CareCove 360° is the continuous management process behind Annual Care.
Every red-flag case receives medical review under CareCove’s medical-governance process, currently led by Dr. Tushar Das.
